Job Details:

Full-time and part-time positions available for key shifts.  Full-time benefits include medical, dental, vision insurance; company matched 401k; paid-time off; company paid life insurance; tuition reimbursement; and eight holidays. Part-time benefits include company matched 401k, paid time off, and an employee assistance program.  Pay rate is $28-30/hr based on experience.

Essential Job Duties:

  1. Administers medication
  2. Administers treatments, including and not limited to dressings, catheterizations, suctioning, oxygen etc.
  3. Assists in direct resident care as needed
  4. Maintains accurate documentation of medication administered and care rendered
  5. Assists in admission and discharge procedures
  6. Responds to emergency call system as indicated
  7. Keeps physician and family aware of changes in resident’s condition
  8. Maintains the self-respect, personal dignity, and physical safety of each patient
  9. Assures continuity of services and care
  10. Assist in the evacuation of residents during an emergency
  11. Supervises nursing personnel Such as GNA and CMT
  12. Attends in-service training and education sessions, as assigned
  13. Familiarity with emergency procedures and may be required to assist with evacuations

Other Duties:

  1. Operate as a member of a team both within the department and across departments.
  2. Performs other work duties and responsibilities as assigned.


  1. Ability to understand and follow instructions in English, communicate effectively, and perform simple arithmetic. Ability to understand measurements and conversions.
  2. LPN currently licensed by the State.
  3. At least 6 months nursing experience preferable in Geriatric Long term care.
  4. Operate as a member of a team both within the department and across departments.
  5. Performs other work duties and responsibilities as assigned.

Physical Requirements/Working conditions:

Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the work day; must be able to lift , transfer, turn and position residents weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally. May be exposed to communicable diseases including the HIV and HBV viruses. Must be able to work paying close attention to detail with frequent interruptions. Requires slip resistant shoes to be worn.
