QI TRACKING CARE COORDINATION ASSISTANT FUNCTIONS:
- To assist the QI /QA/ RM and Tracking Care Coordination Supervisor (hereafter referred to as Supervisor) and Chief Medical Officer (CMO) in QI (Quality Improvement)/ QA (Quality Assurance), RM (Risk Management) and Tracking Care Coordination work;
- To run data reports for ongoing collaboratives, QI projects and programs, PCMH (Patient-Centered Medical Home), CPC (Comprehensive Primary Care), KIDS-CPC (KIDS- Comprehensive Primary Care), UDS (Uniform Data System), IT (Information Technology) and other Center operation functions directly related to QI work.
- Assist in audits as directed.
DUTIES AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:
QI, Collaboratives and Special Projects:
- Assists the QI/QA/RM and Tracking Care Coordination Supervisor (Supervisor) and the Chief Medical Officer (CMO) in QI projects and activities
- Completes data entry and runs reports
- Maintains mailings and making phone calls and other communication to patients for QI purposes
- Compiles and runs reports (patient queries) through the electronic health records, electronic practice management system, and any other health information systems WinMed uses
- Audits charts for compliance; communicates results to QI/QA/RM and Tracking Care Coordination Supervisor and to CMO; assists in formulating remedy actions to the staff, e.g. UDS, PCMH documentation, etc.
- Works with Supervisor in preparing reports or obtaining consultation on reports and systems and communicates problems with IT (Information Technology) to Supervisor
- Attends all QI meetings and assists in taking and completing minutes of QI meetings, submitting them to Supervisor for final review and approval
- Assists with preparation of reports
- Assists with scheduling patients for collaborative/ care coordination services
- Participates in QI/QA/RM and Tracking Care Coordination meetings, conference calls, newsletters, etc.
- Assists with Health Center newsletter and bulletin boards and other communications as indicated
- Assists with center health fairs planning and execution
- Attends learning sessions, conferences, and other outside meetings as requested
- Follows data entry instructions of Supervisor or CMO
- Assists with QI electronic health records activities: such as Patient-Centered Medical Home, CPC, KIDS-CPC and other QI/QA or Risk Management programs that may arise in the future
- Assists with QI programs, such as pertaining to asthma, Centering, diabetes, hypertension, etc.
- Will assists other managers with any special projects, data collection, and compilation, as requested
Risk Management
18. Maintains the Accident-Incident Log of Accident-Incident Reports
19. Assist the CMO in preparation for Risk Management trainings
20. Assist the CMO in Risk Management tasks monitoring and execution
Other Duties:
21. Is responsible for general typing, photocopying, etc. as needed by Administration
22. Is responsible for other duties as assigned.
QI TRACKING CARE COORDINATION ASSISTANT QUALIFICATIONS
- Must posses good clerical and computer skills
- Knowledge of Microsoft Office is important but can be trained in it if not already proficient; especially WORD and Excel.
- Trainable in electronic health records, data entry, data report generation and production
- Must be able to self-initiate work
- Clinical skills and experience are helpful but not required
